Qualifications

• Associate's Degree in a related field and two years of relevant experience.
• Equivalent combination of education and experience.
• Bachelor's degree in social work or related field preferred.
• Experience with data collection and reporting preferred.

Responsibilities

• Coordinate case management services bridging clinical care and community resources.
• Support older adults and families navigating healthcare systems and access community resources.
• Advocate for the needs and rights of older adults within the healthcare system and community.
• Supervise and mentor social work students during field placements and provide guidance.
• Participate in data collection and work with the data team to meet outcomes.
• Deliver dementia care training for healthcare and supportive care workers.
• Monitor and evaluate dementia care program effectiveness and adjust as needed.
• Ensure programs comply with relevant regulations and maintain high-quality care for ADRD.
