Qualifications

• Bachelor's degree in nursing plus four years of related experience.
• Current New York State Registered Nurse license.
• Ability to perform comprehensive patient assessments and coordinate care across settings.
• Strong communication, documentation, and organizational skills.

Responsibilities

• Establish therapeutic relationships with patients, families, and caregivers.
• Provide evidence-based nursing care and care coordination.
• Conduct comprehensive patient assessments through in-person and telehealth encounters.
• Identify high-risk patients and support interventions addressing complex needs.
• Coordinate referrals, diagnostic testing, transportation, and insurance authorizations.
• Facilitate safe transitions across care settings.
• Monitor patient status longitudinally and address barriers to care.
• Educate patients and caregivers regarding diagnoses, treatment plans, and medications.
• Support advance care planning, including MOLST/POLST completion.
• Assist with hospice referrals and bereavement resources.
• Assess caregiver burden and connect with support services.
• Address social determinants of health affecting access and wellbeing.
