Job summary

Millennium Physician Group is hiring a Geriatric Care Coordinator to act as the vital link between our elderly patients, families, and the care team. You will manage appointments, referrals, and follow-ups to ensure seamless navigation and high-quality outcomes.

The role includes administrative support, APCM enrollment, and coordination of home health and community resources, under supervision, in a patient-centered environment.

Qualifications

• Experience in healthcare administrative or patient care coordination is preferred.
• Proficiency with Electronic Health Record (EHR) systems and scheduling platforms.

Responsibilities

• Coordinate, schedule, and confirm patient appointments using Athena and Carelink.
• Serve as primary liaison between patients, families, and care teams.
• Coordinate and schedule post-discharge visits and referrals.
• Enroll eligible patients in APCM and document tasks.
• Track referrals and set up in-person or telehealth visits.
• Screen and return calls for Nurse Practitioner support.
• Perform patient registration and data entry in Athena.
