Job summary

Optimal Care is seeking a Physician Services Clinical Coordinator to serve as the central connection between providers, patients, caregivers, and healthcare partners, ensuring coordinated, compassionate care for elderly and medically complex patients.

You will coordinate services, manage provider orders, facilitate communication between care teams, support patient follow-up, and connect patients with resources needed to support their health and well-being.

Qualifications

• High school diploma or GED is required.
• CENA certification, MA certification, or equivalent work experience.
• Minimum 1 year of experience in medical field or medical office setting.
• Strong grasp of medical terminology.
• Proficiency in Microsoft Office Suite (Outlook, Word, Excel, PowerPoint).
• Reliable transportation with valid automobile insurance.
