Job summary

Millennium Physician Group is seeking a Geriatric Care Coordinator in North Fort Myers, FL to navigate patients through care pathways and coordinate services with the care team.

You will manage appointments, referrals, and post-discharge follow-ups, verify insurance, document interactions in Athena, and support the NP with clinical administrative tasks in a busy geriatric practice. This role requires compassion, strong organization, and excellent communication to enhance patient outcomes.

Qualifications

• Experience in healthcare administrative or patient care coordination preferred.
• Proficiency with EHR systems and scheduling platforms.
• Knowledge of medical terminology and insurance processes.
• Strong organizational and communication skills with compassionate approach.

Responsibilities

• Appointment Management: Coordinate, schedule, and confirm appointments using Athena and Carelink; place reminder calls.
• Liaison/Communication: Communicate with patients, families, providers and care teams.
• Case Management & Referrals: Coordinate post-discharge visits, referrals, and scheduling.
• Program Enrollment: Enroll patients in APCM and document time spent on tasks.
• GUIDE Program Management: Track referrals and setup visits with insurance and location details.
• NP/Provider Support: Screen and return calls to family members on behalf of the NP.
• Administrative & Documentation: Patient registration, census management, insurance verification, and documentation.
• Clinical Support & Processing: Lab/Radiology orders, DME processing, medication assistance, and medical records.
