Job summary

Millennium Physician Group is seeking a Care Coordinator to support an RN Care Manager in outpatient care. This role emphasizes care coordination, patient outreach, and education to improve outcomes and reduce unnecessary utilization.

The candidate may be an LPN or experienced MA, preferably in the Sarasota area. You will work with providers, patients, caregivers, and the care team to ensure continuity of care, track care plans, and help patients navigate community resources.

Qualifications

• Minimum of 3 years of clinical healthcare experience in a physician practice, outpatient clinic, population health, care management, or related setting.
• Experience with chronic disease management and high-risk patient populations.
• Proficiency with electronic health records (EHR).
• LPN license or CMA/RMA certification/licensure (Florida).

Responsibilities

• Assist the RN Care Manager in implementing and monitoring individualized patient care plans.
• Perform monthly patient chart reviews to identify care gaps and preventive needs.
• Monitor and track follow-up appointments, referrals, testing, and care plan goals.
• Coordinate services across primary care providers, specialists, hospitals, and community resources.
• Conduct telephonic outreach to patients/caregivers and assess barriers to care.
• Support transition-of-care activities after ED visits or hospitalizations.
• Maintain accurate documentation in the EHR and manage medical records.
