Job summary

South Shore Health System is seeking a Case Manager Specialist to coordinate care across the system, working with RN Case Managers, Social Work, and care teams to connect patients with community resources and post-acute services. The role emphasizes professional communication, timely outreach, and effective referrals to homecare and post-acute facilities.

The specialist will document activities in the EMR, manage referrals for post-acute vendors, and maintain strong collaboration with the care

Qualifications

BS in Psychology, Social Work, Communications or health related field preferred.
3-5 years recent healthcare experience or related field preferred.
Experience working with patients and families over the phone.

Responsibilities

Coordinate care coordination and transitional care interventions for South Shore Health populations.
Conduct outreach calls and document activities via patient outreach.
Arrange transportation and escalate issues to RN Case Manager as needed.
Create referrals for Post-Acute care needs (Rehab, SNF, Homecare).
Maintain up-to-date communication with care team and patients/families.
Document referrals and communications in the EMR for review by clinical team.
