Job summary

Advocate Health seeks a Case Manager to develop, implement, and evaluate care coordination for defined patient populations. You will work under the Director of Coordinated Care to optimize outcomes and advocate for patients through a collaborative care team.

The role requires RN licensure in the applicable state, with BSN preferred and Case Management certification encouraged. 40-hour week, with a Monday–Friday schedule, located in Winston-Salem, NC.

Qualifications

• Graduate of an accredited School of Registered Professional Nurses, BSN preferred, MSN encouraged
• Minimum of five years of experience in the care of assigned patient population
• Current registration to practice as a Registered Nurse in the applicable state; Case Management certification encouraged

Responsibilities

• Educate, consult, and assist in the management of an identified patient population to achieve high-quality outcomes
• Collaborate daily with the patient care team including physicians, nurses, therapists, pharmacists, and other disciplines
• Advocate, coordinate, and communicate to ensure continuity and cost-effective care
