Job summary

CURATUSSC is seeking a qualified Case Manager/Nurse to coordinate comprehensive resident care within the facility. The role requires a Bachelor’s degree in Nursing or Social Work, with RN license preferred, and CCM certification a plus.

Two years of clinical nursing experience and knowledge of Medicare/Medicaid programs are expected. You will collaborate with residents, families, providers, and payers to optimize care and costs.

Qualifications

• Bachelor's Degree in Nursing or Social Work and RN license preferred.
• Two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal and Medicaid programs and benefits.
• PCC knowledge preferred.
• CCM certification a plus.
• Must maintain continuing education/licensing.
• Must remain in good standing with the Department of Public Health, License and Certification Division.

Responsibilities

• Utilize clinical expertise and critical thinking to develop and implement a plan of care.
• Communicate with residents and families about plans of care and treatment protocols.
• Attend morning meetings to facilitate team communications.
• Answer residents' questions about care and illness progression.
• Monitor and adjust resident statuses based on needs.
• Coordinate admissions, discharges, referrals with staff and third parties.
• Complete documentation in a timely manner.
• Perform utilization review to ensure cost-effective care.
• Maintain accurate charting and paperwork.
