You will assess risk, develop care plans, monitor outcomes and coordinate with caregivers, physicians and community resources to support members with complex conditions.

Qualifications

• Proficiency in Microsoft Word, Excel and Outlook.
• Experience in analyzing trends with decision support systems.
• Strong business management and cost-analysis skills.

Responsibilities

• Facilitates a team approach with the Interdisciplinary Care Plan team to ensure quality and cost-effective care.
• Collaborates with care providers, family and legal representatives to address member needs.
• Conducts in-depth health risk assessments and comprehensive needs assessments.
• Provides care coordination to members with chronic or complex conditions.
