Responsibilities

• Supports patients in hospital, inpatient or clinic settings.
• Facilitates interdisciplinary care planning to ensure quality care.
• Coordinate care for chronic conditions and transitions to home care.
• Assess plan of care, identify gaps, and adjust interventions as needed.
• Educate providers and families about care coordination and resources.
