Job summary

Wellstar Cobb Hospital is seeking an RN Senior with strong care coordination and discharge planning skills to oversee transitional care and discharge planning for complex patients. You will collaborate with physicians, care teams, and social work to develop effective post-acute plans and optimize LOS.

Ideal candidates bring 3+ years in acute care, 2+ years in care coordination, and experience mentoring staff. Relocation assistance is available for eligible candidates within the Wellstar system.

Qualifications

• Minimum 3 years of healthcare experience in acute care or related field.
• Minimum 2 years in care coordination in acute care setting.
• Strong clinical expertise to partner with physicians and care teams.

Responsibilities

• Assess transitional care needs and coordinate across the continuum.
• Engage with patient and family to assure care needs are met.
• Mentor new hires in clinical progression and discharge planning.
• Collaborate with utilization review, physicians and care team.
• Manage discharge planning and post-discharge referrals.
• Document timely patient progress and discharge plans.
