Job summary

Wellstar Health System’s Cobb Hospital seeks an experienced RN Sr Care Coordinator to assess transitional care needs, coordinate across the continuum, and engage with patients and families to meet complex needs during the acute admission. The RN Sr partners with physicians and care teams to drive efficient treatment plans and discharge readiness.

Qualifications

• Associates Nursing or Diploma in Nursing or Bachelors Nursing-Preferred.
• Minimum 3 years of experience in healthcare in acute care, related field, skilled care or community environment in care coordination.
• Minimum 2 years in care coordination in the acute care setting.

Responsibilities

• Assess psychosocial risk factors and resources to support discharge.
• Collaborate with physicians and care team to plan discharge and care progression.
• Identify barriers to discharge and work with leaders for timely resolution.
• Document clinical and psychosocial assessments in the medical record.
• Precept and mentor new hires; participate in interdisciplinary rounds.
• Support professional development and ensure compliance with policies.
