Job summary

Qualifications

• Associates Nursing or Diploma (Nurse) required; Bachelor's preferred.
• RN license (Single State or Compact) required.
• Minimum 1 year nursing experience in acute care.

Responsibilities

• Assess transitional care needs and coordinate across the care continuum.
• Plan and document discharge plans with patients, families, and care teams.
• Facilitate interdisciplinary rounds and care conferences.
