Job summary

Wellstar Cobb Hospital is seeking a Care Coordinator RN Sr to assess transitional care needs and coordinate care across the continuum during acute admission. You will partner with physicians and the care team to drive efficient treatment plans and discharge planning.

The role requires strong clinical expertise, precepting duties, and collaboration with the care team to optimize patient progression and resource use. Relocation assistance may be available for eligible candidates.

Qualifications

• Minimum 3 years of healthcare experience in acute care, related field, skilled care or community environment in care coordination.
• Minimum 2 years in care coordination in the acute care setting required.

Responsibilities

• Assess transitional care needs and coordinate across the continuum.
• Engage with patient and family to ensure care needs are met during acute admission.
• Mentor new hires and precept students as needed.
• Document and track discharge planning and care progression.
