Job summary

Wellstar Cobb Hospital is seeking a Care Coordinator RN Sr. to assess transitional care needs, coordinate across the care continuum, and engage patients and families to meet complex discharge goals. The role partners with physicians and care teams to streamline treatment plans and promote efficient resource use.

Responsibilities include interdisciplinary rounds, discharge planning, and mentoring new staff, with a focus on patient-centered care during acute admission and post-discharge planning.

Qualifications

• Minimum 3 years of healthcare experience in the acute care setting or related field in care coordination.
• Minimum 2 years in care coordination in the acute care setting.
• Relocation assistance for eligible candidates.
• Hours: FT Days, 12 hour shifts covering the Emergency Department.

Responsibilities

• Assess transitional care needs and coordinate care across the continuum.
• Engage patient and family to assure care needs are met in complex cases.
• Partner with physicians and care teams to drive efficient treatment plans.
• Lead discharge planning and coordinate post-discharge care.
