Job summary

Wellstar Health System in Georgia is seeking a Care Coordinator RN Sr to assess transitional care needs, coordinate care across the continuum, and engage patients and families to meet complex goals during acute admission.

The RN Sr partners with physicians and care teams to drive efficient treatment plans, support discharge planning, and mentor new hires while participating in rounds to ensure safe transitions.

Qualifications

• Minimum 4 years of experience as a staff nurse in acute care.
• Minimum 2 years of hospital-based case management experience.
• Current Georgia RN license or license via eNLC.
• BLS certification required on hire.

Responsibilities

• Assess psychosocial and discharge planning needs.
• Coordinate care progression and post-discharge transitions.
• Participate in interdisciplinary rounds and care conferences.
• Mentor and precept new hires and staff.
• Advocate for timely, efficient patient flow and resource use.
