Job summary

ChenMed LLC in St. Louis is seeking a Patient Continuity and Needs Advocate (PCNA) to bridge patients with health plans and support services, ensuring eligible individuals receive tailored care and resources.

You will work with center leadership, brokers and clinical staff to boost retention, re-engage disenrolled members, ensure care continuity, and document patient interactions in EMR, Medals and Salesforce with HIPAA compliance.

Qualifications

High School diploma or GED required; BA/BS preferred or equivalent experience.
Minimum 3 years of work experience; senior care exposure is a plus.
Experience with Medicare Advantage is a plus.

Responsibilities

Advocate and connect patients with health plans and community resources.
Proactively reach out to disenrolled members to re-engage care.
Collaborate with center leadership, brokers and clinical staff on retention.
Ensure seamless care continuity and prevent gaps in care.
Document patient issues and resolutions in EMR, Medals and Salesforce with HIPAA.
