Job summary

ChenMed in the United States is seeking a Patient Continuity and Needs Advocate (PCNA) to bridge patients with health plans and support services tailored to their needs, especially seniors. You will collaborate with center leadership, sales teams, brokers, and clinical staff to develop retention strategies, ensuring seamless care transitions and reducing disruptions.

Expertise in EMR systems, Salesforce, Microsoft Office, and strong communication is expected, with commitment to HIPAA compliance

Qualifications

• High school diploma or GED required
• BA/BS degree preferred or equivalent experience considered
• Minimum of three years’ work experience; Medicare Advantage knowledge a plus

Responsibilities

• Advocates for patients and connects eligible individuals with health plans and resources
• Retention and re-engagement: outreach to disenrolled members to facilitate return to care
• Collaborates with center leadership, sales teams, brokers and clinical staff to drive retention strategies
• Ensures care continuity and reduces gaps in care
• Documents patient information in EMR systems and Salesforce; maintains patient accounts
• Complies with HIPAA and regulatory requirements
• Performs other duties as assigned by management
