Job summary

Qualifications

• Must reside in Illinois or a border state within a commutable distance of IL.
• Minimum Associates nursing degree required.
• 3+ years of hospital or equivalent care setting experience.
• 2+ years in care management and one year acute care experience required.

Responsibilities

• Coordinate care for members with acute, chronic, behavioral health needs to ensure access to services and outcomes.
• Perform telephonic and/or face-to-face assessments and care planning across the lifespan.
• Identify barriers, monitor progress, and collaborate with providers and community resources.
• Participate in interdisciplinary care team meetings and ensure regulatory compliance.
