Job summary

Curana Health is seeking a Care Ally, Case Manager to join our interdisciplinary care team, coordinating health and social needs for members. You will liaise with members, RP/POAs, and care teams to plan, implement, monitor, and adjust care plans while guiding families through benefits and services.

The role requires clinical practice in case management, strong communication, and the ability to travel 2–3 days per week with a home-office component. RN/LPN or SW background is preferred.

Qualifications

• One year of clinical practice in case management, home health, discharge planning or related fields.
• Proficiency with Microsoft Office (Word, Excel, Outlook) and navigating multiple systems.
• Case management certification preferred; licensure may be required as RN/LPN.
• Long-term care or nursing facility experience preferred.

Responsibilities

• Execute strategies to improve overall Member experience across the ICT.
• Contact plan members to assess health and develop a care plan with the ICT.
• Educate members and families about disease status, plan benefits, and resources.
• Collaborate with care team and medical director to coordinate treatments.
• Follow up with members to monitor progress toward health goals.
• Coordinate care plans and document care team meetings for Plan communication.
• Assist with locating in-network providers and scheduling appointments.
