Job summary

Curana Health is hiring a Care Ally, Case Manager to join an interdisciplinary care team. The role includes assessing needs, planning care, coordinating with family/POAs, and collaborating with the medical director to ensure quality, patient-centered care for seniors.

Ideal candidates have 1 year of relevant clinical practice, case management certification preferred, and license to practice (RN/LPN). Travel to facilities 2–3 days weekly with hybrid work from a home office.

Qualifications

• One year of clinical practice in case management, home health, critical care, med/surg, discharge planning, or concurrent review.
• Case management certification preferred.
• Long-term care or nursing facility experience preferred.
• Proficiency with Microsoft Office and navigating multiple systems.

Responsibilities

• Execute strategies to manage and improve member experience.
• Conduct health assessments and develop care plans with the interdisciplinary care team.
• Educate members and families on disease status, plan benefits, and resources.
• Collaborate with medical directors to ensure appropriate treatment.
• Follow up with members to assess progress and update care plans.
• Coordinate with RP/POAs and care teams; assist with appointments and benefits.
• Support home health coordination, durable medical equipment, and therapy visits.
• Document care team meetings and transmit updates to Plan.
