Job summary

Curana Health, Inc. is seeking a Care Ally, Case Manager to join our interdisciplinary care team. You will assess member health and social needs, develop and monitor care plans, and coordinate with the member, RP/POA, and care team to ensure optimal outcomes.

Preferred candidates hold an RN/LPN license or a BSW/MSW, with experience in case management, home health, or discharge planning. This role offers hybrid work, requires travel to facilities, and focuses on high-quality, patient-centered

Qualifications

• 1 year of clinical practice in case management, home health, or related areas.
• Proficiency in Microsoft Office (Word, Excel, Outlook).
• Case management certification preferred.
• Long-term care or nursing facility experience preferred.

Responsibilities

• Executes strategies to improve member experience as directed by leadership.
• Contacts plan members to assess health and develop a care plan with the interdisciplinary team.
• Educates member, family, and caregiver about disease status, plan benefits, and resources.
• Collaborates with medical directors to facilitate appropriate treatment for members.
• Follows up with members to assess progress toward health goals.
• Communicates health updates to RP/POAs and coordinates care plan updates.
