Job summary

Curana Health, Inc. is seeking a Care Ally, Case Manager to join the interdisciplinary care team. You will assess needs, plan and coordinate care, and liaise with Members, RP/POAs, and care providers to ensure high-quality, person-centered services.

The role emphasizes collaboration, education, and proactive care coordination across facilities, with a hybrid work arrangement and a focus on improving health outcomes for older adults in senior living communities.

Qualifications

• One year of clinical practice experience in case management, home health, critical care, medical/surgical, discharge planning, or concurrent review.
• Active, unrestricted RN or LPN license in the state where job duties are performed; OR Bachelor’s degree in social work (BSW). A Master of Social Work (MSW) and licensure are preferred but not required.
• Proficiency using Microsoft Office (Word, Excel, Outlook) and navigating multiple systems.

Responsibilities

• Execute on strategies and goals for managing and improving overall Member experience.
• Conduct comprehensive health assessments and develop plans of care with the interdisciplinary care team.
• Educate members and caregivers about disease status, plan benefits, and resources.
