Job summary

Curana Health, Inc. is seeking a Care Ally, Case Manager to join our interdisciplinary team. You will conduct comprehensive health assessments, plan and coordinate care, educate members and families, and work with care teams across facilities to optimize health outcomes and member satisfaction.

The role requires an RN/LPN license or a BSW/MSW, with field visits 2–3 days weekly and hybrid work from home. Experience in case management and long-term care is preferred; sponsorship is not available.

Qualifications

• One year of clinical practice in case management, home health, critical care, medical/surgical, discharge planning, or concurrent review.
• Proficiency using basic Microsoft Office (Word, Excel, Outlook) and ability to navigate multiple systems.
• Case management certification preferred.
• Long-term care or nursing facility experience preferred.

Responsibilities

• Execute strategies and goals to improve overall Member experience.
• Contact Plan members to assess health and develop plans of care.
• Educate members and families as health coaches on disease status and plan benefits.
• Collaborate with care team and medical director to ensure appropriate treatment.
• Follow up with members to monitor progress toward goals.
• Develop health goals with members and arrange interventions.
• Coordinate appointments, care, and utilization of benefits.
• Support Members, RP/POAs and communities with locating in-network providers.
• Assist with home health and therapy visits and equipment coordination.
• Document care team meetings and update care plans as needed.
