Job summary

Curana Health, Inc. is seeking a Care Ally, Case Manager to join our interdisciplinary care team. You will assess member health and social needs, plan care, coordinate services, and liaise with RP/POAs, PCPs, and care partners.

This hybrid role requires travel to facilities 2–3 days per week, strong organizational skills, and a clinical background such as RN/LPN or BSN with case management experience; MSW preferred.

Qualifications

• One year of clinical practice in case management, home health, critical care, medical/surgical, discharge planning, or concurrent review.
• Case management certification preferred.
• Long-term care or nursing facility experience preferred.

Responsibilities

• Executes care strategies and goals set by leadership to improve member experience.
• Conducts comprehensive health assessments and develops plans of care with the interdisciplinary team.
• Educates members and families about disease status, plan benefits, resources, and options.
