Job summary

JASA in Brooklyn, NY is seeking a Case Manager to provide clinical and practical support to older adults and their families through in-home and in-office assessments, under supervision of Master’s-level staff.

You will develop tailored care plans, coordinate services (homecare, meals, transportation, legal aid), screen for benefits, and advocate to secure entitlements such as Medicare and Medicaid. The role also includes organizing daily community activities and supervising staff.

Qualifications

• Graduate with a BSW/BA/BS in social work or equivalent.
• Ability to conduct comprehensive assessments and document findings.
• Strong written communication and data organization skills.

Responsibilities

• Conduct comprehensive in-home and in-office assessments of clients' social and emotional needs.
• Develop and implement written case plans; provide counseling to preserve client autonomy.
• Screen clients for entitlement eligibility and advocate to secure benefits (Medicare, Medicaid, public assistance).
• Coordinate direct services including homecare, transportation, meals on wheels, legal aid, and emergency care.
• Evaluate client capacity to manage affairs and assist with guardianship or relief fund disbursements.
• Organize daily community activities and monthly events; facilitate group counseling and caregiver support groups.
• Supervise community aides, Title V workers, homemakers, home health aides, interns, and volunteers.
• Maintain electronic case records, reports, and statistics; represent JASA at meetings.
