Job summary

JASA is seeking a Case Manager to support older adults at the Co-Op City Center network. You will connect participants with benefits, services, and programs both in person and remotely, assess needs, assist with applications, and advocate for access to resources.

Under supervision of the Program Director, you will maintain confidential records, coordinate transportation, homecare, meals, housing, and other services, and collaborate with community partners to improve health, independence, and

Qualifications

• Graduate with B.S.W., B.A., or B.S. degree.
• Prior case management or social services experience preferred.
• English reading/writing and strong communication skills required.

Responsibilities

• Provide Information and Referral and Case Assistance to participants.
• Assist with applications for benefits and entitlements and coordinate services.
• Advocate for participants to obtain appropriate services and supports.
• Maintain complete case records and prepare reports; use secure databases.
• Respond to crises and participate in trainings and staff meetings.
