Qualifikationen

• Graduate of an accredited college or university with a BSW or BA/BS (Psychology or Gerontology preferred).
• Computer literacy and the ability to maintain meticulous records.
• Proficiency in English required; knowledge of Yiddish, Russian, Spanish, or Patois helpful.
• Ability to travel to clients’ homes using mass transit and navigate stairs.
• Sensitivity to race, religion, ethnicity and sexual orientation in serving a diverse client base.

Aufgaben

• Crisis Triage & Intake: conduct screenings and triage for emergency interventions; provide immediate information and referrals.
• Comprehensive Assessment: in-home and in-office assessments of social and emotional needs using standardized instruments.
• Case Planning & Implementation: develop written case plans including counseling, benefit screening, and arranging services.
• Financial & Legal Protection: evaluate capacity to manage affairs; assist in securing guardianship when needed.
• Field Monitoring: ongoing in-home visits to monitor risk, evaluate functioning, and revise plans.
• Emergency Response & On-Call: rotating on-call schedule; respond to crisis situations requiring action.
• Documentation: complete case records, reports, and statistics within mandated timeframes.
