Job summary

Jewish Association For Services For The Aged (JASA) in New York, NY is seeking a Case Manager for Adult Protective Services. You will handle crisis intervention, protective services, and social casework for vulnerable seniors, both in crisis and in ongoing planning.

This full‑time role requires strong judgment and a commitment to elder justice. Responsibilities include crisis triage, comprehensive assessments, and coordinating services (Home Health Aides, Legal Aid, Housing).

Qualifications

• Graduate with an accredited degree in social work or related field.
• Knowledge of elder protection laws and social casework principles.
• Ability to conduct risk assessments and crisis triage.

Responsibilities

• Crisis triage and intake for emergency interventions and referrals.
• Comprehensive in-home and office assessments of social needs.
• Develop written case plans including counseling and service coordination.
• Evaluate capacity for financial/guardianship decisions and protect seniors.
• Conduct ongoing home visits, reassess plans, and adjust accordingly.
• Respond to on-call shifts for evenings, weekends, or holidays.
• Document case records and statistics in required software.
