Job summary

SCAN Health Plan is seeking an Advanced Practice Clinician to provide primary care to SCAN members in their residences, typically in senior living or skilled nursing facilities.

The role includes exams, vaccines, testing, goals-of-care discussions, and coordination with case management to prevent hospitalizations. Travel is frequent and good communication is essential for high-quality, patient-centered care.

Qualifications

• Master’s degree in Nurse Practitioner Certificate OR Master’s from an accredited PA program, Education in Adult/Geriatric medical care preferred.
• Oregon Nurse Practitioner license - OR- Oregon Physician Assistant Certification (PA-C) in good standing.
• Wound Care Certificate and Palliative Care Certificate a plus.
• Knowledge of CMS guidelines and Medicare Advantage Managed Care.
• Adaptable and open to change; strong communication and care coordination skills.

Responsibilities

• Provide care to an assigned panel of members in their place of residence (senior living communities or SNFs).
• Conduct physical/wellness exams and assessments; administer vaccines; order and interpret diagnostic tests.
• Develop a comprehensive plan of care with the patient, family, and staff.
• Collaborate with case management to ensure timely access to services.
• Travel 75%+ of the time in the assigned area; coordinate with care teams to ensure continuity.
