Job summary

Claremedica Health Partners in Tampa, FL seeks an Access Community Benefits Coordinator to help patients access essential social service programs. You will navigate Medicaid, SNAP, LIHEAP, and other supports while advocating for patients and tracking outcomes.

Responsibilities include eligibility screenings, applications, orientation, and collaboration with clinical teams to coordinate wraparound services and reduce barriers to care.

Qualifications

• Experience with DCF Medicaid, SNAP, SSA/SSI, Medicare, Section 8, LIHEAP, STS and related community programs.
• Customer service skills and client-facing experience.
• Proficiency in data entry and MS Office (Word, Excel, Outlook).
• English, Spanish and Creole language skills a plus.
• Strong verbal and written communication; ability to juggle multiple tasks.

Responsibilities

• Assist patients with Medicaid redeterminations and recertifications; ensure timely submissions.
• Screen eligibility for Medicaid and Dual Eligibility to maximize benefits.
• Identify and support growth of Dual Eligible members at centers.
• Conduct new member orientations on available benefits and services.
• Lead generation and outreach to refer patients to sales/enrollment teams.
• Document interactions and outcomes in EHR; maintain compliance.
