Job summary

Claremedica Health Partners is seeking an Access Community Benefits Coordinator to connect patients with essential social services and navigate Medicaid, SNAP, Lifeline, and related programs. You will collaborate with center teams to ensure wraparound support and improve health outcomes.

The role requires strong customer service, data entry, and knowledge of public assistance programs, with documentation in the EHR and regular outreach activities.

Qualifications

• Experience with DCF Medicaid, SNAP, and other programs.
• Strong data entry and MS Office skills (Word, Excel, Outlook).
• Excellent communication and customer service abilities.
• Ability to manage multiple projects and meet deadlines.

Responsibilities

• Assist patients with Medicaid redeterminations and recertifications.
• Screen eligibility for Medicaid and Dual Eligibility (Medicare & Medicaid).
• Identify and support Dual Eligible members to improve access to benefits.
• Provide application assistance for SNAP, LIHEAP, Lifeline, LTC Waiver, STS and related programs.
• Document interactions and outcomes in the EHR and internal systems.
