The role emphasizes patient advocacy, eligibility screening, and accurate documentation in EHRs, with a focus on reducing barriers to care and improving health outcomes.

Qualifications

• Experience with DCF Medicaid & SNAP, Social Security SSA - SSI, Retirement, Disability and Medicare, Section 8 Housing, LIHEAP, STS and additional Community Social Programs.
• English, Spanish and Creole speakers a plus.
• Demonstrable ability to communicate, build trust and rapport with clients on the phone.
• Data Entry and Experience with MS/Office (Word, Excel & Outlook) required. Advanced analytics reporting via Excel.

Responsibilities

• Medicaid Recertifications: Assist patients with DCF Medicaid redeterminations and recertifications, ensuring documentation is submitted on time to avoid lapses in coverage.
• Eligibility Screening: Conduct thorough screenings of all members for Medicaid eligibility and Dual Eligibility (Medicare & Medicaid).
• Dual Member Growth: Identify and support eligible patients to increase Dual Eligible members at each center.
• New Member Orientations: Conduct orientations to educate patients about available benefits and access methods.
• Lead Generation & Outreach: Generate at least 10 qualified Access leads per month and refer to sales/enrollment teams.
• Access Engagement Rate: Ensure 90% of center members are engaged with an Access Representative.
• Application Assistance: Provide direct support with applications for SNAP, LIHEAP, Lifeline, LTC Waiver, STS, etc.
• Documentation & Reporting: Maintain accurate records in internal systems.
• Team Collaboration: Coordinate with clinical teams, care managers, and outreach staff.
• Documentation in EHR: Record daily appointments and services in the EHR.
