Job summary

Claremedica Health Partners, LLC is seeking an Access Community Benefits Coordinator in Orlando, FL. The role helps patients access government and community programs, guiding them through Medicaid, SNAP, LIHEAP, Lifeline and other benefits, while coordinating with care teams to remove barriers to care.

Responsibilities include eligibility screening, Medicaid redeterminations, new member orientations, outreach to generate leads, and meticulous documentation in the EHR system to support quality

Qualifications

• Experience with DCF Medicaid & SNAP, SSA - SSI, Retirement, Disability and Medicare, Section 8 Housing, LIHEAP, STS and additional Community Social Programs.
• Data Entry and MS Office proficiency required.
• English, Spanish and Creole speakers a plus.

Responsibilities

• Medicaid recertifications: assist patients with DCF Medicaid redeterminations and recertifications.
• Eligibility screening: conduct screenings for Medicaid eligibility and Dual Eligibility (Medicare & Medicaid).
• Dual member growth: identify and support eligible patients to increase Dual Eligible members.
• New member orientations: educate patients about benefits and access.
• Lead generation & outreach: generate a minimum of 10 qualified Access leads per month.
• Access engagement rate: ensure 90% of members are engaged and seen by an Access Representative.
• Application assistance: help with SNAP, LIHEAP, Lifeline, LTC Waiver, STS, etc.
• Documentation in EHR: record all interactions and outcomes.
• Team collaboration: coordinate with clinical teams and outreach staff to wrap services.
