Job summary

Claremedica in Florida is seeking an Access Community Benefits Coordinator to connect patients with essential social service programs and resources. You will partner with center staff to navigate Medicaid, SNAP, LIHEAP, and other services, helping patients understand eligibility and complete applications.

This role emphasizes outreach, documentation in the EHR, and collaboration with care teams to reduce barriers to care and improve health outcomes for seniors in our communities.

Qualifications

• Experience with DCF Medicaid & SNAP, Social Security SSA - Supplemental Security Income SSI, Retirement, Disability and Medicare, Section 8 Housing, LIHEAP, STS and additional Community Social Programs.
• Customer Services Skills
• Data Entry and Experience with MS/Office (Word, Excel & Outlook) required. Advanced analytics reporting via Excel.
• English, Spanish and Creole speakers a plus.
• Demonstrable ability to communicate, build trust and rapport with clients on the phone.
• Proven ability to juggle multiple projects at a time, while maintaining sharp attention to detail.

Responsibilities

• Medicaid & SNAP program eligibility support and recertifications.
• Screening for Medicaid eligibility and Dual Eligibility (Medicare & Medicaid).
• Lead generation and outreach to increase program engagement.
• Document and report on interactions and outcomes; ensure compliance with procedures.
• Coordinate with clinical teams and outreach staff to provide holistic support.
