Boston Children’s Hospital is seeking a Patient Navigator to support behavioral health staff through case management and care coordination. You will assist with referrals, scheduling, transportation, insurance advocacy, and education for patients transitioning to adult care.

The role requires flexibility, strong communication, efficient documentation, and on-site presence at least three days weekly. BA and/or health care client-facing experience is preferred, with BSN/BSW as an option.

Formación

• BA and/or experience with patient care in a community or health setting.
• Ability to manage difficult conversations and prioritize tasks by urgency.
• Willingness to learn and on-site requirement (at least three days a week).
• Strong documentation and organizational skills.

Responsabilidades

• Meet with or call patients and families to review ongoing care plans.
• Explain hospital/community resources and assess referrals.
• Act as liaison with internal and community resources.
• Coordinate scheduling and transportation; assist with insurance applications.
• Provide insurance information to care teams to determine eligibility.
• Develop and maintain relationships for accessing resources.
• Document patient care and collect required data; track outcomes.
• Support planning processes and education for staff and patients.
