Qualifications

• Two years of related behavioral health experience; or equivalent combination of education and experience.
• Experience coordinating services with schools, families, and providers.

Responsibilities

• Deliver intensive outpatient case management in school settings.
• Coordinate care with schools, families, and external providers.
• Maintain timely, accurate documentation and data collection.
• Provide crisis intervention and outreach for missed appointments.
• Conduct home visits and community outreach as needed.
