ESSENTIAL JOB DUTIES:
- Organize and plan workload in an efficient manner
- Operate and understand computer and computer systems (SAMS, Mobile Assessments, ETC.)
- Communicate effectively orally and in writing
- Follow laws, county and agency policy in conducting work including but not limited to confidentiality and chain-of-command
- Successfully complete state required trainings
- Successfully test for any certification/credentialing
- Prepare for and participate in staff and unit meetings
- Determine eligibility for various programs
- Select/interpret/apply policy/procedures/regulations
- Conduct interactive interviews
- Interact objectively with consumers/staff
- Perform basic mathematical calculations
- Operate various office machines
- Walk distances and climb stairs
- Adapt to changing regulations, procedures and priorities
- Keep detailed records
- Ability to lift 25 pounds
- Obtain training to develop knowledge and skills related to work tasks
- Travel throughout the county to complete fieldwork, including to areas without public transportation
GENERAL AGENCY RESPONSIBILITIES:
- Provides care management services to consumers and members of their families to improve quality of life
- Is familiar and collaborates with community professionals, community agencies, and other community resources that will contribute to meeting the needs of consumers
- Assists older adults in developing or maintaining personal autonomy and identifying and providing resources to address their social, medical, and emotional needs
- Has working knowledge of the PA Department of Aging program policies, procedures, and requirements
- Communicates effectively orally and in writing
- Conducts home visits with older adults, caregivers and family members as necessary
- Ability to establish and maintain effective, professional working relationships
- Completes assessments, care plans, and/or referrals for resources
- Documents case activity in PA Department of Aging computer system
- Is responsible for protecting the security of all personal health information (oral or recorded in any form) that is obtained, handled, learned, heard, or viewed in the course of their work or association with the Agency
- Completes cross-training requirements and demonstrates the ability to implement knowledge in other departments within the Agency
- Attends agency staff meetings and trainings as directed by Supervisory staff, including trainings provided by PDA for the purpose of maintaining Protective Service Investigator certifications
- Organizes and plans workload efficiently
- Applies critical thinking skills to identify and resolve problems
- Has knowledge of current social, economic, and health issues that impact the aging population
- Operates office equipment including computer, copier, fax, mobile wi-fi device, cell phone, and multiple phone lines
- Performs basic mathematical calculations
- Works overtime as determined necessary and with Supervisory approval
- Participates in special project teams as designated
- Works with student interns as assigned by Supervisory staff
- Works in cubicle office environment
- Walks distances and climb stairs
- Travels throughout the county to complete fieldwork, including to areas without public transportation
- Performs all other related work as required
Work is assigned by supervisor and is supervised by daily observation, annual evaluations of care managers, case reviews and consultation with supervisor as needed.
Minimum Experience and Training Requirements
- Six months as an Aging Care Manager I or a County Caseworker I; or
- Successful completion of the County Social Casework intern program; or
- Six months of professional human services or nursing experience; and a bachelor's degree in social services, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training.
