Job summary

The Commonwealth of Pennsylvania is seeking an Aging Care Manager 2 for the Monroe County Area Agency on Aging in Stroudsburg. You will assess aging consumers, develop care plans, and coordinate with community resources to help older adults address personal, social, and financial needs.

This full-time position involves home visits, documentation in SAMS/PIA, and providing benefit assistance. A six-month equivalent or bachelor's in a related field is required; telework may be possible after

Qualifications

• Six months as Aging Care Manager 1 or County Caseworker 1 or equivalent experience.
• Six months of professional human services or nursing experience.
• Bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field.
• An equivalent combination of experience and training.

Responsibilities

• Consumer assessment to identify needs and locate services.
• Care management to support consumers and families.
• Home contacts via visits and calls per plans and policies.
• Complete assessments and maintain records in SAMS and PIA systems.
• Benefit assistance and information on relevant laws.
• Coordinate with community resources to access services.
