Job summary

Community Partnership of Southeast Missouri (CPSEMO) is seeking a compassionate Case Manager for SEMO RISE, a region-wide workforce development initiative serving 14 counties in Southeast Missouri. You will assess participant needs, develop service plans, coordinate referrals, monitor progress, and provide ongoing support to help individuals engage in education, training, and employment.

Qualifications

• Associate or bachelor’s degree in social work, human services, counseling, psychology, workforce development, education, or a related field.
• At least one year of experience in case management or workforce development.
• Strong interpersonal, communication, and problem-solving skills.
• Ability to assess needs, coordinate referrals, and support progress toward goals.
• Strong organizational skills and the ability to maintain accurate case documentation.
• Ability to manage multiple participants and competing priorities.
• Proficiency with Microsoft Office and electronic recordkeeping systems.
• Ability to maintain confidentiality and handle sensitive information.
• Ability to collaborate with community organizations and service providers.
• Ability to travel throughout the 14-county service area and have a valid driver’s license.

Responsibilities

• Perform initial and ongoing participant assessments and develop individualized service plans.
• Coordinate referrals to education, training, and employment resources.
• Monitor progress, maintain documentation, and report outcomes.
• Provide barrier-reduction supports such as transportation or childcare as allowed by program guidelines.
• Engage with partners and training providers to support retention and completion.
