Job summary

Sagility seeks a Senior Care Specialist (Registered Nurse) to work from home on a seasonal assignment ending June 2027. The role focuses on telephonic health assessments, developing actionable care plans, and coordinating remote care for Medicare, Medicaid, and group health plan members.

You will collaborate with a multidisciplinary team, provide health coaching, document interactions accurately, and support transitions of care including DME and home health services.

Qualifications

• Active, unrestricted RN license in the state of residence.
• Compact RN license permitting practice in applicable states.
• Minimum 2 years of clinical nursing experience; 3+ preferred.
• Strong clinical judgment, critical thinking, and problem-solving.

Responsibilities

• Conduct telephonic health assessments for members.
• Develop actionable, individualized care plans.
• Establish health coaching relationships and provide education.
• Use nursing judgment to coordinate interventions for acute situations.
• Support transitions of care and discharge planning with DME/home health.
• Help members navigate benefits and care options.
• Assess environmental and psychosocial factors affecting health.
• Collaborate with multidisciplinary teams.
• Promote self-management with education and resources.
• Coordinate care activities among members, caregivers, and providers.
• Document interactions, assessments, plans, and outcomes accurately.
