Sagility is seeking an experienced Registered Nurse to join a Senior Care Specialist team in a seasonal, work-from-home capacity. The RN will conduct telephonic health assessments, develop care plans, and coordinate transitions of care for Medicare, Medicaid, and group health plan members.

The role requires an active RN license and compact licensure where applicable, with 2–3+ years of clinical nursing experience. Strong judgment, communication, and remote work proficiency are essential.

Formación

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently in a remote environment.
• Proficient with MS Word, Excel, and Outlook.

Responsabilidades

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop individualized care plans based on member needs and health risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Coordinate transitions of care and arrange DME, home health, and provider appointments.
• Help members navigate healthcare benefits and care options.
• Assess factors affecting health and well-being.
• Collaborate with multidisciplinary teams.
• Promote self-management with education and resources.
• Coordinate care and manage conditions among care team members.
• Document interactions, assessments, and outcomes accurately in systems.
