Job summary

Adult & Child Health in Indianapolis seeks a School Based Support Specialist to provide intensive outpatient case management for youth in school settings, coordinating care with schools, families, and community partners to support academic success.

You will deliver individualized behavioral health interventions, carry a caseload, monitor progress, and participate in staff development, with flexibility to meet evolving student needs.

Qualifications

• Experience delivering case management in mental health or substance use settings.
• Ability to coordinate services with schools, families and partners.
• Knowledge of treatment planning and crisis intervention.

Responsibilities

• Provide behavioral health interventions aligned with treatment goals in school settings.
• Coordinate care with schools, families, and service providers.
• Manage a caseload with timely documentation and follow-up.
• Provide crisis intervention and outreach for missed appointments.
• Conduct home visits and community outreach as needed.
• Participate in QA activities and staff development.
