Job summary

Adult & Child Health is seeking a School Based Support Specialist to deliver intensive outpatient case management for youth in school settings. You will provide individualized interventions, coordinate care with schools and families, and support academic and community functioning during school hours with targeted home/community outreach as needed.

Qualifications

• Bachelor's degree in psychology, social work, behavioral health, or related human services field, OR relevant 2+ years experience.
• Experience coordinating with schools and community partners preferred.
• Knowledge of confidentiality and ethical standards in behavioral health.
• Ability to document services timely and accurately in electronic records.

Responsibilities

• Provide school-based behavioral health interventions aligned with treatment plans.
• Support students' coping skills, social-emotional functioning, and daily living.
• Coordinate care with schools, families, and service providers.
• Manage an assigned caseload and ensure timely, person-centered documentation.
