Job summary

Adult & Child Health seeks a School Based Support Specialist to deliver intensive outpatient case management for youth in schools, supporting success at school, home, and in the community. You will provide direct interventions, coordinate care with schools and families, and promote recovery and resilience during school hours with additional support as needed.

You will work to strengthen academic achievement and daily living skills, handle a caseload, and participate in ongoing staff development

Qualifications

• Must have a degree in psychology, social work, behavioral health or related field, or equivalent experience.
• Experience delivering one-on-one or group services focused on recovery and resiliency.
• Experience in school, community, or employment functioning and treatment planning.

Responsibilities

• Provide school-based behavioral health interventions aligned with treatment goals.
• Support coping skills, social-emotional functioning, and daily living skills.
• Coordinate care with schools, families, and providers.
• Maintain timely documentation and manage a caseload.
• Provide crisis intervention and follow-up, including outreach for missed appointments.
