Job summary

Centerstone seeks a School Based Support Specialist to deliver intensive outpatient case management for youth within a school setting. You will support academic, home, and community success through individualized interventions, care coordination, and recovery-focused planning.

The role involves school-based services during school hours with additional home/community support as needed. Strong communication, documentation, and crisis intervention skills are essential for success in this position.

Qualifications

• Bachelor's degree in psychology, social work, behavioral health, or related field.
• 2 years of related behavioral health experience if bachelor’s in unrelated field.
• Any four-year combination of higher education and two years of qualifying experience.
• AS in behavioral health support or equivalent.
• Four years of full-time qualifying experience in the field.

Responsibilities

• Provide intensive outpatient case management in a school setting.
• Coordinate care with schools, families, and providers.
• Maintain timely documentation and data collection.
• Deliver crisis intervention and follow-up.
• Conduct home visits and community outreach as needed.
• Participate in staff development and supervision.
• Adhere to agency protocols and quality assurance.
