Job summary

Adult & Child Health is seeking a School-Based Support Specialist to deliver intensive outpatient case management for youth in Franklin - Northwood. Services occur in schools during school hours, with occasional home and community visits to support treatment plans and recovery goals.

You will coordinate care with families, schools, and external providers, maintain timely documentation, and provide crisis intervention when needed.

Qualifications

• Bachelor’s degree or equivalent in human services as stated.
• Experience in school-based or youth behavioral health preferred.
• Ability to work with diverse families and schools.

Responsibilities

• Provide behavioral health interventions aligned with treatment goals in school-based settings.
• Support students’ coping strategies, social-emotional functioning, and daily life skills.
• Participate in care coordination with schools, families, and providers.
• Manage a caseload, keep timely documentation, and ensure services are person-centered.
• Provide crisis intervention and follow-up, including outreach for missed appointments.
• Conduct home visits and outreach to support youth and family engagement.
• Assist with quality assurance, data collection, and compliance with agency protocols.
• Participate in staff development, supervision, and evaluations.
• Adapt to evolving student needs and support program goals with collaboration.
• Other duties as assigned.

Bachelor’s degree in an unrelated field with two years of related behavioral health experience

Four years of full-time qualifying experience in the field
