Job summary

Center for Human Development in Greenfield, MA is seeking a part-time School-Based Community Health Worker to join our Community School Based Clinic in Franklin County. The role supports students and families by coordinating care and linking them with tele behavioral health services and community resources.

You will document encounters, collect data, complete monthly reports, and assist with referrals and releases of information.

Qualifications

High School Diploma or Equivalent required.
Strong computer skills - EMR databases.
Knowledge of area community agencies and resources.
Skill in organizing resources and establishing priorities.
Creative and analytical thinking.
Strong interpersonal and communication skills.

Responsibilities

Provide home, school and community-based care coordination to youth and their families.
Assist students and families to access on site tele behavioral counseling services as well as offsite community-based clinicians and non-clinical resources (as needed).
Conduct comprehensive screening/assessment activities including mental health status, health screen and social needs factors, to support community-based referrals.
Provide ongoing follow-up, motivational interviewing and goal setting with students/families.
