Jane Pauley Community Health Center, Inc. is seeking a Family Nurse Practitioner (FNP) to join our Medical Services team in Indiana. The provider will deliver comprehensive primary care within an FQHC setting, focusing on prevention, diagnosis, and management of acute and chronic conditions.

Ideal candidates hold an FNP degree, board certification, prescriptive authority, and strong collaboration skills, with experience in team-based care and serving diverse populations.

Qualifications

• Master’s or Doctoral degree in Nursing from an accredited program.
• Board certification as a Family Nurse Practitioner (FNP).
• Prescriptive authority and DEA licensure (or ability to obtain).
• Strong clinical assessment, diagnostic, and decision‑making skills.
• Excellent communication and collaboration skills.

Responsibilities

• Conduct patient assessments, physical exams, and diagnostic evaluations.
• Diagnose and manage acute and chronic conditions across the lifespan.
• Develop and implement individualized treatment plans.
• Prescribe medications and perform procedures within scope of practice.
• Provide preventive care, screenings, and health maintenance services.
